The patient-centered medical home, a model currently being tested in a multi-payer pilot that includes Anthem Blue Cross and Blue Shield, creates a collaborative healthcare setting between the patient and physician, designed to explain the role of specialists, provide information on appropriate and compassionate treatment options, answer questions about illnesses and coordinate care ordered by specialists.  The PCMH encompasses information on how to access care for all stages of life, including acute and chronic care, preventive services and end-of-life care.  PCMHs empower consumers to access healthcare options appropriate to their particular conditions and make healthier lifestyle choices.  Multi-payer pilots of the model are also being developed by Anthem Plans in Maine, New Hampshire, New York and Ohio.  Additional programs are being discussed and considered in Missouri, California and Wisconsin.
